Why Your Elderly Parent Is Not Sleeping Well
Why Your Elderly Parent Is Not Sleeping Well — And What You Can Do About It
Sleep Problems in Seniors Are Extremely Common — and Often Fixable
Amma wakes up at 2am every night and cannot go back to sleep.' 'Thatha falls asleep during the day and then is wide awake at midnight.' 'My mother has not had a full night's sleep in months.'
If any of these sound familiar, you are far from alone. Sleep disturbances affect up to 50% of elderly people — but they are not simply an inevitable feature of aging. In most cases, they have identifiable causes and effective solutions.
How Sleep Changes as We Age
Normal age-related changes in sleep include:
• Earlier sleep onset and earlier waking — the circadianrhythm shifts forward with age
• More time in lighter sleep stages and less in deep, restorative sleep
• More frequent nighttime awakenings
• Increased daytime sleepiness
These changes are normal in themselves. Problems arise when they are compounded by medical conditions, medications, poor sleep hygiene, or environmental factors — turning manageable changes into genuinely disruptive sleep disorders.
Common Causes of Sleep Problems in Elderly People
Medical Conditions
Pain conditions (arthritis, back pain), breathlessness (heart failure, COPD), nocturia (frequent urination at night), restless legs syndrome, and sleep apnoea are all major disruptors of elderly sleep. Treating the underlying condition often dramatically improves sleep.
Medications
Many common medications disrupt sleep — including some blood pressure drugs, diuretics (particularly when taken in the evening), certain antidepressants, and steroids. A medication review by the treating physician can often identify and resolve medication-related sleep issues.
Depression and Anxiety
Both depression and anxiety profoundly disrupt sleep. Early morning waking (waking 2–3 hours before desired and being unable to return to sleep) is a classic symptom of depression in older adults. Addressing the mental health condition often resolves the sleep problem.
Poor Sleep Hygiene
Irregular sleep times, excessive daytime napping, too much time in bed while awake, using the bedroom for non-sleep activities, and stimulating evening activities (overly exciting TV, phones) all undermine sleep quality.
Why Poor Sleep Is Serious in Seniors
Chronic poor sleep in elderly people is associated with:
• Accelerated cognitive decline and increased dementia risk
• Increased fall risk — fatigue affects balance and reaction time
• Worsened cardiovascular health
• Depressed immune function
• Mood deterioration — irritability, anxiety, and depression
• Reduced quality of life and daily functioning
Practical Steps to Improve Sleep in Seniors
• Establish a consistent sleep and wake time — even on weekends
• Limit daytime naps to 20–30 minutes before 3pm
• Create a calming bedtime routine — warm bath, light reading, prayer, gentle music
• Ensure the bedroom is dark, quiet, and comfortably cool
• Avoid tea or coffee after 2pm
• Treat nocturia — speak to the doctor about timing of fluid intake and diuretics
• Increase daytime physical activity and sunlight exposure
• Review all medications with the doctor for sleep-disrupting side effects
A Note on Sleep Medications in Seniors
Sleeping tablets (sedative-hypnotics) should be used with extreme caution in elderly patients. They increase fall risk dramatically, can cause next-day confusion, and often worsen sleep quality over time. They should only be used under close medical supervision, at the lowest possible dose, for the shortest possible duration.
In most cases, non-medication approaches — sleep hygiene, treating underlying conditions, and cognitive behavioural therapy for insomnia (CBT-I) — are safer and more effective.
