When the same area of skin is pressed against a surface for too long, blood supply is cut off and the tissue breaks down. Bedsores can develop within hours on bony prominences like the heels, hips, tailbone, and shoulder blades. They are painful, slow to heal, and prone to infection.
Prevention: Turn and reposition your loved one every 2 hours. Use a pressure-relief mattress. Keep skin clean and dry. Inspect skin daily for early redness.
Muscle Atrophy and Contractures
Muscles that are not used shrink and weaken rapidly. Joints can become stiff and locked in position (contractures) if not moved regularly. This makes rehabilitation harder and pain more likely.
Prevention: Perform passive range-of-motion exercises — gently moving arms, legs, and joints through their natural range — at least twice daily. A physiotherapist can teach you the correct technique.
Respiratory Complications
Lying flat for extended periods reduces lung capacity and makes it harder to clear mucous, increasing the risk of pneumonia. Encourage deep breathing exercises. Where possible, elevate the head of the bed to 30 degrees.
Urinary Tract Infections and Constipation
Immobility slows digestion and urinary flow. Ensure adequate hydration, fibre in the diet, and proper hygiene during toileting or catheter care.
Daily Care Essentials
• Oral hygiene: Clean teeth and mouth twice daily even if the person cannot eat orally
• Skin care: Moisturise daily, especially elbows, heels, and back
• Hair and grooming: Regular grooming preserves dignity and mood
• Nutrition: Small, frequent meals; consult a dietitianfor bed-bound nutritional needs
• Hydration: Offer fluids every 1–2 hours; dehydration is common and dangerous
• Bowel and bladder: Monitor output daily; constipation or UTI symptoms need prompt attention
The Emotional Side of Bedridden Care
A bedridden person is still fully present emotionally and intellectually. Many experience profound frustration, grief, and loss of identity — particularly if they were previously active and independent. This emotional suffering is real and deserves acknowledgment.
Speak to them, not about them. Include them in family conversations. Ask their opinion. Read to them. Play their favourite music. Maintain eye contact and gentle touch. These small acts of connection are deeply healing.
